Clinic Dates: 1st and 4th Saturday of every month from 8:00 a.m. - noon. How do I make an appointment? Please call the clinic office 503-831-0551 to make an appointment. Walk-ins are accepted if space is available basis. People are seen on a first come, first serve basis. Once all the appointments are taken, then you will have to wait for the next clinic. Who is eligible? Adults who have no health insurance. Where is the clinic located? At Trinity Lutheran Church, 320 SE Fir Villa Rd., Dallas, Oregon (Across the street from the drive-in theater.) Why? Doctors, nurses and many volunteers from Polk County have seen the desperate need for basic health care for non-insured citizens. They have responded by giving of their time without getting paid.
